Overview

 

The Bi-lingual Case Manager provides case management for approximately 35 individuals with HIV/AIDS according to the standards established for Title XIX case management. 

 

SCOPE OF THE WORK

 

The Generalist Case Manager, under the supervision of the Client Services Director is responsible for the following:

 

1.      Conduct screening intakes, comprehensive assessments and develop service plans with PCAF clients to address their health care and psycho-social needs.

2.      Monitor, review and update the service plan with the client on scheduled basis tailoring plan to clients identified needs. 

3.      Document client contacts as required by funding sources.  Maintain accurate records and prepare written statistical reports as required being done accurately and in a timely manner.

4.      Develop and maintain knowledge of HIV/AIDS related conditions and treatment and use knowledge to educate clients and their support systems.

5.      Develop and maintain positive working relationships with medical providers and health and social service providers; participate in case reviews with other agency staff and PCAF staff. 

6.      Refer clients to appropriate providers of needed services identified in the service plan.

7.      Develop and maintain a thorough knowledge of eligibility and benefits provisions of public and private financial assistance and health care coverage programs.

8.      Educate clients in the concept of harm reduction.

9.      Develop and maintain positive working relationships with other Spanish speaking services providers including the Immigrations and Naturalization Service (INS).  Work collaboratively with others in the system to identify service gaps (especially for undocumented persons) and develop or expand resources to fill those gaps.

10. Perform other duties as assigned.

 

QUALIFICATIONS

1.      Bachelor’s degree required in psychology, social work or closely related filed from a fully accredited college or university; Master’s degree preferred.  Employment history must include a minimum of two (2) years related work in a health or human service agency.

2.      Experience working with individuals with mental illness, chemical dependency, the dually diagnosed and the homeless. 

3.      Experience providing social services to monolingual Spanish-speaking persons. Experience working with undocumented persons desired.

4.      Experience working with HIV – infected individuals. 

5.      Knowledgeable about the grieving, death and dying process.   Experience working with people who are very ill and/or in the process of dying. 

6.      Demonstrated ability to work effectively under stressful conditions with people of varying socioeconomic backgrounds, sexual orientations and ethnic diversity.

7.      Excellent written and verbal communication skills.  Group training and facilitation experience is preferred.

8.      Knowledge of community resources and referrals.

9.      Willingness and ability to articulate and abide by PCAF’s philosophy and policies in providing service to clients and in representing the agency.

10. Ability to be flexible, supportive and to work cooperatively with staff as a member of a diverse team.

11. Proficiency in computer word-processing and excel.  Prefer ability to access information via the Internet.

 

SALARY AND BENEFITS

The salary for this position is $32,000 per annum.  Liberal benefits include medical, dental, vision, life and long-term disability insurance and an employee assistance program, plus paid vacation and sick leave.
